Freelancer’s Union is the best I’ve found. It’s a kind of a pain to get enrolled, but it’s pretty affordable (relatively). Good luck!

I’ve tried ehealthinsurance.com in the past, with decent results. Their site and phone support was pretty informative. Wow, sounds like an ad, but seriously, I don’t get a cut.
